Annual Data Checks and Medical/Health Forms

Today your child should have brought home forms for completion.

Annual Data Check, we use this information to ensure that the information we hold is up to date, including addresses, phone numbers and details of who can contact in an emergency e.g.. if you child is unwell.  Please return this as your earliest convenience. 

If your child has a health condition or medical need, please complete the Medical/Health Forms and return as soon as possible.  These forms require updating annually to ensure we have up to date information about children’s needs. 

Once these have been returned, the office staff will be in touch with the parents of any child who has a health need to discuss whether he/she requires/continues to require medication in school or can self-carry.  Self-carrying is where a child has parental consent to take medication independently.  Although given the age and stage of primary pupils, we do not encourage this.  You may be asked to provide us with a Health Care Plan for conditions such as asthma or diabetes.  These are NHS plans and should be available from your child’s GP, practice nurse or consultant.
